• Transitional assistance from hospital to home
• Care Management to personalize your care
• Bathing, Dressing, Grooming, Toileting and Oral Care
Companionship
• Food shopping, Meal Preparation and Feeding
• Light Housekeeping
• Telehealth Services
• Accompaniment to medical and community appointments

Financing options

Medicaid Waivers
Non-medical home care support services are available to disabled and aging clients, ages 18 and over who are eligible to participate in the following programs:

Waiver Programs including:

• Community Health Choices
• Act 150
• OBRA
• PFDS
Consolidated and Community Living

Or, PA Home and Community-Based Services (HCBS).

Private Pay

The following services are available to private pay clients:

• Personal care assistance
• Respite care
• Services
• Community integration
• Insurances
• In addition, Veterans Assistance, Workers Compensation and Long Term Care Insurances are accepted.
